Please note that my in-network provider status with these insurance companies is subject to change. 

I's highly recommended that you contact your insurance company directly to confirm benefits and eligibility. Some insurance companies offer out-of-network benefits that can apply to psychotherapy services.  Also, some employers offer flexible spending accounts that can cover costs of services.   I will provide you with a receipt which you may submit to your insurance company or flexible spending account representative. It is your responsibility to contact your insurance company or employer to get details as to your out-of-network coverage. Please feel free to contact me to discuss your insurance coverage in more detail or with any other questions regarding payment or fees.  

Out of Network & Private Pay fees:

Initial Evaluation: $225

Individual: $175 / 45 minutes

                   $225 / 60 minutes

Couples: $275 / 60 minutes

Family: $275 / 60 minutes

All copays and fees are due at the time of service.  Sliding scale options may be available, please inquire!
